Job summary

Molina Healthcare is seeking a Care Manager for LTSS (Remote) to support care management and coordination for high-need members across Wayne County and surrounding areas. You’ll work with multidisciplinary teams to coordinate services via LTSS and waiver programs, ensuring progress toward outcomes.

The role involves assessments, waiver enrollment, care-plan development, monitoring, and collaboration with providers. Travel is 25-40% as required by state contracts.

Qualifications

• At least 2 years of health care experience with LTSS and care management experience.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of community resources and ability to navigate portals/databases.
• Ability to work independently with minimal supervision and stay organized.
• Strong verbal and written communication skills, including documentation.
• Proficiency with Microsoft Office and online portals/databases.
• In some states, experience with substance use disorders is required.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor and adjust care plans, document interventions and outcomes.
• Promote integration of LTSS, behavioral health, and home/community resources.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and advise on funding sources.
• Lead ICT meetings for service approvals/denials and collaboration.
• Use motivational interviewing and Molina guidelines to educate and support change.
• Identify barriers to care and coordinate psycho/social/financial support.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required by state/contractual requirements.
