Job summary

You will collaborate with a multidisciplinary team to coordinate member care, develop and monitor care plans, and ensure progress toward outcomes. Travel 25-40% locally may be required based on state requirements.

The role emphasizes assessing medical necessity, coordinating waiver services, and educating members using motivational interviewing techniques to support positive health changes.

Qualifications

• 2+ years in health care, with care management or LTSS experience.
• Active, unrestricted license in state of practice (LCSW/LMSW/etc.).
• Valid driver's license and reliable transportation.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans in collaboration with members, caregivers and clinicians.
• Monitors care plans for effectiveness and documents progress.
• Promotes integration of LTSS with behavioral health and community resources.
