Job summary

Molina Healthcare is seeking a Care Manager to support LTSS care coordination for high-need members. You will conduct comprehensive assessments, develop and monitor care plans, and coordinate waiver enrollment with a multidisciplinary team to achieve desired outcomes.

This role requires travel 25-40% and a background in healthcare with LTSS or disability services. Strong communication, organizational skills, and ability to work independently are essential. A valid driver’s license is necessary.

Qualifications

• 2+ years healthcare experience with LTSS/care management or equivalent.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care plans including waiver service plans.
• Monitor care plans for effectiveness and adjust as needed.
• Coordinate waiver enrollment/disenrollment processes.
• Promote integration of LTSS with behavioral health and community resources.
• Collaborate with ICT meetings for approval/denial of services.
• Travel up to 25-40% as required.
