Job summary

Molina Healthcare is seeking a Care Coordination Specialist to support LTSS programs and member care across the continuum. The role collaborates with a multidisciplinary team to ensure high-need members progress toward outcomes and cost-effective care.

Responsibilities include assessing, planning, monitoring, and coordinating waiver services, with anticipated 25–40% local travel. Previous LTSS or disability services experience is preferred; a valid driver's license is required where applicable.

Qualifications

• 2+ years healthcare experience with LTSS or disability services.
• Valid driver's license and reliable transportation.
• Ability to work independently in diverse settings.
• Willingness to travel 25-40% as required.
• Licensure required only if state contract demands.

Responsibilities

• Complete comprehensive member assessments within timelines.
• Develop and implement care plans with stakeholders and member network.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
• Collaborate with care teams and leadership.
• Travel 25-40% based on state/contractual requirements.
