Job summary

Molina Healthcare is seeking a remote field-based Care Manager to support long-term services and supports (LTSS) and waiver care in the greater Cook County area. This role requires travel and collaboration with an interdisciplinary team to coordinate member care and achieve desired outcomes.

The RN will assess, plan, implement and monitor care plans, promote service integration, and navigate waivers. Travel up to 40% may be required; strong communication and problem-solving are essential.

Qualifications

• RN with active, unrestricted license in state of practice.
• At least 2 years in health care with 1 year in care management or LTSS.
• Valid driver's license, reliable transportation and auto insurance.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and others.
• Monitors care plan effectiveness and documents outcomes, adjusting as needed.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Assesses medical necessity and authorizes waiver services.
• Participates in ICT meetings for approvals/denials.
• Uses motivational interviewing and Molina guides to educate and motivate change.
• Identifies barriers and coordinates resources to address psycho/social, financial, and medical concerns.
• May provide medication reconciliation and consult with peers; travel 25–40%.
