Job summary

Molina Healthcare is seeking a field-based care coordination professional to support LTSS members across the continuum. This role involves remote work with substantial travel and collaboration with multidisciplinary teams to ensure high-quality, cost-effective member care.

The position emphasizes comprehensive assessments, waiver enrollment/disenrollment, and care plan development, with travel 25-40% as required by state contracts. Strong communication and problem-solving skills are essential.

Qualifications

• At least 2 years health care experience including LTSS and care management.
• Valid driver's license, reliable transportation, adequate auto insurance for travel.
• Bachelor's degree in health care field may be required depending on state.
• License must be active/unrestricted if required by state.
• In some states, must have experience with individuals with substance use disorders.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Develops and implements care plans including a waiver service plan with members, caregivers, physicians and others.
• Monitor care plan to evaluate effectiveness and document interventions and outcomes.
• Coordinate services to enhance continuity of care across behavioral health and LTSS.
• Assess medical necessity, authorize waiver services, and evaluate benefits.
• Facilitate ICT meetings for approval/denial of services.
• Travel 25-40% may be required depending on state/contractual requirements.
