Job summary

Qualifications

• At least 2 years of health care experience with LTSS, care management, or related field.
• Licensure/certification only if required by state contract or law and active.
• Bachelor’s degree in a health care field may be required in some states.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, families, and providers.
• Monitor care plans and document progress toward goals.
• Coordinate services across behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services.
• Explain benefits and funding sources to members.
• Lead ICT meetings for service approvals/denials.
• Support member education and motivate change during contacts.
• Identify barriers and coordinate resources to address obstacles.
• Identify incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required by contract.
