Job summary

Centene Corp. is seeking a LTSS Service Care Coordinator to support remote Illinois members, with 50-75% local travel within specified counties. The role focuses on holistic care management, patient/family education, and care coordination with providers to enable quality, cost-effective outcomes.

Requires a Bachelor's degree and 1 year of related experience, or Illinois RN/LPN-holders or human services degrees per the IL plan criteria. Travel and regulatory compliance are key aspects of the role.

Qualifications

Bachelor's degree and 1 year related experience or equivalent.
For Illinois plan: RN Illinois license or Bachelor/Master’s in human services.
One of the IL criteria must be met for eligibility.

Responsibilities

Evaluates member needs, resources, and recommends the best care plan.
Develops ongoing long-term care/service plans and identifies providers and resources.
Coordinates between member/family/caregivers and care provider team.
Provides resource support for employment, housing, independent living, and other needs.
Monitors plans, member status, and outcomes; offers recommendations.
Interacts with healthcare providers and partners to meet needs.
Collects and maintains member information to ensure regulatory compliance.
May perform on-site visits and provide education to members and families.
Provides leadership with feedback to improve quality and cost-effectiveness.
