Job summary

The role requires about 75% travel to member homes, development of care plans with providers, and ongoing monitoring of outcomes. A Bachelor's degree and related experience are required, with RN/LPN licensure preferred for Illinois plans.

Qualifications

• Requires Bachelor's degree and 1 year related experience or equivalent.
• RN license in Illinois for Illinois plan or equivalent licensure.
• LPN with experience in comprehensive assessments preferred.

Responsibilities

• Evaluate member needs, resources, and plan outcomes.
• Develop long-term care plans and identify providers and resources.
• Coordinate between member/family and care team to access services.
• Provide resource support for employment, housing, and living needs.
• Monitor plans, member status, and outcomes; adjust as needed.
• Interact with long-term care providers and partners to meet needs.
• Collect and maintain member information for regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Educate members and families on procedures, options, and benefits.
• Provide feedback to leadership on quality improvement opportunities.
• Perform other duties as assigned; comply with policies and standards.
