Job summary

Tuesday Health is hiring a Licensed Practical Nurse (LPN) in Lynn, Massachusetts. The role focuses on delivering coordinated, compassionate care for members with serious illnesses, working with nurse practitioners and RNs to assess, plan, and execute individualized care plans.

The LPN will participate in rounds, use electronic systems, and engage with members to build trust and deliver a person-centered experience. Valid MA LPN license and driving ability are required.

Qualifications

• Massachusetts LPN license active and unrestricted.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care model preferred.
• Strong communication and organization skills.
• Ability to learn new electronic documentation systems.

Responsibilities

• Administer assessments and screenings with support from RNs/NPs.
• Prioritize needs based on assessments and coordinate with the care team.
• Develop and execute individualized care plans with the team.
• Participate in care rounds to ensure a coordinated member experience.
• Learn Tuesday Health electronic systems and tools.
• Build trusting relationships with members through listening and empathy.
• Act as quarterback of the multidisciplinary team to transform care.
