Job summary

Tuesday Health in Woburn, MA is seeking a Licensed Practical Nurse to join our multidisciplinary team delivering coordinated, compassionate care for patients with serious illnesses. The LPN will perform clinical assessments, support care plans, and ensure clear communication across the care team.

You will work closely with nurse practitioners and registered nurses, participate in rounds, and use our electronic tools to enhance the member experience.

Qualifications

• Licensed Practical Nurse licensed in Massachusetts (MA) without any board action.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model with network providers and care management.
• Strong communication and organization skills.
• Ability to multitask and maintain a positive member experience under pressure.
• Know when to elevate and act in emergent situations.
• Strong technical skills to navigate documentation systems and learn new systems as the company grows.
• Valid driver’s license with a safe driving record.
• State minimum automobile insurance coverage.
• Comfort with change and rapid evolution of the business.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model with NP/RN support.
• Prioritize needs based on assessments and coordinate with the interdisciplinary team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build a strong, trusting relationship with every member through listening and empathy.
• Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
