Cadence Health is seeking a remote Licensed Practical Nurse (Clinical Navigator) to provide virtual support for Medicare patients with chronic conditions. In this full-time role, you will deliver patient-centered outreach, ensure adherence to care plans, and educate patients on health management strategies.

The ideal candidate must hold an active compact multi-state LPN/LVN license and have at least 5 years of clinical experience. The compensation range is $26 – $31 per hour, with various benefits including competitive pay and a remote work environment.

Qualifications

• Must have an active compact multi-state LPN/LVN license.
• 5 years of clinical experience as a Licensed Practical Nurse required.
• Stable high-speed WiFi for remote work is necessary.

Responsibilities

• Conduct outreach to patients enrolled in Cadence care programs.
• Provide coaching and support to patients managing chronic conditions.
• Document patient interactions and outcomes in the Cadence platform.
