Job summary

Cadence is seeking a Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. The role involves patient outreach, coaching, and collaboration with clinical teams to enhance patient care outcomes.

Successful candidates will hold an active LPN/LVN license, have at least 5 years of clinical experience, and be ready to work remotely. The compensation is anticipated to be $26 – $31 per hour, along with comprehensive benefits.

Qualifications

• An active compact multi-state LPN/LVN license is required.
• At least 5 years of clinical experience as a Licensed Practical Nurse is required.
• Remote work readiness (including stable high-speed Wi-Fi) is required.
• Experience in patient education for chronic conditions is preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching.
• Help patients execute personalized care plans.
• Conduct comprehensive assessments of patients’ health statuses.
• Educate patients on disease management and medication adherence.
• Document patient interactions and monitor progress.
