Qualifications

• Nursing license (RN/LPN) or medical assistant with 2+ years in care coordination.
• Experience with injury case management and coordination of care.
• Strong communication and organizational skills.

Responsibilities

• Analyze and review work-related injuries and coordinate care plans.
• Conduct initial referral calls outlining needs and care steps.
• Coordinate services and arrange transportation as needed.
• Monitor case progress and advocate for services and entitlements.
• Collaborate with internal departments to ensure timely discharge.
