Job summary

One Call is seeking an internal case management support professional to ensure the best continuum of care for injured workers. You will collaborate with all business units to provide timely services at optimal cost, supporting the carrier and coordinating with physicians until case closure.

The role emphasizes coordination, communication, and analytical review of cases, with an emphasis on customer service and teamwork in a remote-friendly environment.

Qualifications

• RN/LPN or MA with at least two years of experience or equivalent.
• Knowledge of care coordination and injury management processes.
• Excellent communication and organizational skills.

Responsibilities

• 70% Analysis and review of work-related injuries, including assessment, planning, coordination and evaluation.
• Initial referral review with overview of needs, plan of care and steps to mitigate health care needs.
• Coordinate services, arrange transportation and escort as needed.
• Monitor cases, verify attendance and advocate for needed services.
• Collaborate with One Call departments to ensure timely services and cost-effective care.
• Develop and implement discharge plans and post-discharge resources.
• Contribute to process improvements and reporting on treatment results.
