Job summary

National Church Residences seeks an LPN – Advanced Practice to coordinate care across nursing facilities, provide telephonic support, and educate patients and families. The role reports to the VP of Operations, Primary Care, with remote support responsibilities.

You will review care plans, assist with referrals, document in the EHR, and ensure regulatory compliance. Strong communication, computer skills, and English proficiency are essential; travel is rare.

Qualifications

• Must hold a current LPN license in good standing in Ohio.
• Minimum of 2 years of clinical nursing experience in long-term care, case management, or related settings.
• Telehealth or remote nursing experience preferred.

Responsibilities

• Coordinate care with nursing facilities and providers; address clinical inquiries.
• Provide telephonic support to patients and families; educate on care needs and concerns.
• Act as liaison among healthcare providers, nursing facilities, patients.
• Review and update patient care plans with facilities and providers; monitor progress.
