CareHarmony, Inc. is seeking a licensed practical nurse (LPN/LVN) to join our remote care coordination team. This full-time, work-from-home role focuses on value-based care management and improving patient outcomes.

You will identify resources, coordinate needs for chronic care patients, educate on disease management, and assist with referrals, scheduling, and medication reconciliation. A multi-state LPN license and strong communication are essential.

Qualifications

• Active LPN license in multi-state or compact license required.
• 3+ years of direct patient-facing work experience.
• Strong communication skills and attention to detail.
• Proficiency with Microsoft Office Suite and healthcare software.

Responsabilités

• Manage patient census and close care gaps in clinical and non-clinical care.
• Identify and coordinate community resources for chronic care patients.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management, reconciliation, and coordination of refills.
• Assist with timely delivery of services (Home Health, DME, Home Infusion).
• Support referrals and appointment scheduling.
