Job summary

National Church Residences is seeking an LPN - Advanced Practice to coordinate care across nursing facilities, provide telephonic support to patients and families, and maintain accurate EHR records.

You will liaise with providers and facilities, review care plans, and assist with referrals and transitions of care while adhering to HIPAA and organizational policies. Remote support capabilities are utilized where appropriate.

Qualifications

• Education: Degree in nursing field as a licensed practical nurse.
• Experience: Minimum of 2 years clinical nursing experience in long-term care, case management, outpatient settings, or similar environment. Telehealth or remote nursing experience preferred.
• Travel: Rare 0-24%
• Mental: Must have excellent communication, comprehension, and interpersonal skills. Must have the ability to speak, read, write and understand English, as well as the ability to make self-understood.
• Skills: Must have strong working computer skills with basic professional software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.
• Licensure: Must have a current LPN license in good standing in the state(s) in which practicing.
• Vision: Normal: Consistent with standard workflow.

Responsibilities

• Coordinate patient care plans with nursing facilities.
• Provide telephonic support to patients and families.
• Act as a liaison between healthcare providers, nursing facilities, and patients.
• Review and update patient care plans in collaboration with nursing facilities and providers.
• Monitor patient progress and ensure continuity of care.
• Assist with arranging referrals, home healthcare, or specialized services as needed.
• Maintain accurate and detailed patient records in the electronic health record (EHR) system.
• Document telephonic interactions, care plans, and follow-up actions per policy.
• Educate patients and caregivers on medication management and chronic condition care.
• Assist with scheduling appointments and transitions of care.
• Complete pre-authorizations and communicate with insurance providers.
• Monitor compliance with regulatory standards.
• Identify and address barriers to care; escalate as needed.
• Participate in on-call rotation.
