Job summary

CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) to work remotely with care teams to support value-based care initiatives.

The ideal candidate has at least 3 years of direct patient-facing experience, holds an active Minnesota LPN license, and demonstrates strong communication. You will manage patient needs, coordinate resources, educate patients, handle medication management, and assist with referrals and scheduling.

Qualifications

• Active Minnesota LPN license is required.
• Minimum of 3 years direct patient-facing experience.
• Must thrive in a fast-paced environment and be self-motivated.
• Excellent written and verbal communication skills are essential.

Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in care.
• Identify and coordinate community resources for patients.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management including reconciliation and refills coordination.
• Ensure timely delivery of services (Home Health, DME, Home Infusion, etc.).
• Resolve patient questions and maintain open dialogue to understand needs.
• Assist/manage referrals and appointment scheduling.
