Job summary

National Church Residences in Columbus, OH is seeking an LPN - Care Coordinator to coordinate care across facilities, educate patients, and support families by phone.

The role acts as liaison between providers, facilities, and patients; you will review care plans, arrange referrals, document in the EHR, and monitor progress to ensure continuity of care.

Qualifications

• Ohio LPN licensure in good standing.
• Minimum of 2 years clinical nursing experience in long-term care, case management, or related settings.
• Strong computer skills and ability to learn new software quickly.
• Self-directed with excellent organizational abilities and task management.

Responsibilities

• Coordinate patient care plans with nursing facilities and address clinical inquiries.
• Provide telephonic support to patients and families, educate on care needs and concerns.
• Act as liaison between healthcare providers, facilities and patients.
• Review and update patient care plans, monitor progress and ensure continuity of care.
• Assist with arranging referrals and home healthcare, document in EHR, manage appointments.
