Job summary

CareHarmony is seeking an experienced Licensed Practical Nurse (LPN/LVN) to join our remote care coordination team. This full-time role supports chronic care management by coordinating resources and educating patients while maintaining HIPAA compliance and accurate documentation.

You will work 100% remotely from home with a Monday–Friday schedule, no weekends, and an occasional on-call rotation. Compensation starts at $21/hour and can go up to $28/hour based on production and experience.

Qualifications

• Active LPN/LVN license in a multi-state/compact status.
• Experience delivering chronic care management and patient education.
• Proficient with Microsoft Office and strong communication skills.

Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
• Identify and coordinate community resources with patients that would benefit their care.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management including reconciliation, adherence, and coordinating refills.
• Assist in ensuring timely delivery of services to patients (Home Health, DME, Home Infusion, etc.).
• Support referrals and appointment scheduling.
