Job summary

National Church Residences is seeking an LPN for the Advanced Practice role in Senior Services. You will coordinate care across nursing facilities, provide telephonic support to patients and families, and act as a liaison between healthcare providers, facilities, and patients.

Responsibilities include updating care plans, monitoring patient progress, arranging referrals, documenting in the EHR, educating on medications and self-care, and assisting with scheduling and transitions of care.

Qualifications

• LPN license in good standing in the applicable state(s).
• Minimum 2 years clinical nursing experience in long-term care or similar setting.
• Telehealth or remote nursing experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinate patient care plans with nursing facilities.
• Provide telephonic support to patients and families.
• Liaise between healthcare providers, nursing facilities, and patients.
• Review and update care plans with facilities/providers.
• Monitor patient progress and follow up to ensure continuity of care.
• Arrange referrals, home healthcare, or specialized services as needed.
• Maintain accurate patient records in the EHR system.
• Educate patients and caregivers on medication management and self-care.
• Assist with scheduling appointments and transitions of care.
• Complete pre-authorizations and communicate with insurers.
• Monitor regulatory compliance and address barriers to care.
• Participate in on-call rotation.
