CareHarmony, Inc. is hiring a Licensed Practical Nurse (LPN/LVN) to work 100% remotely in a full-time role.

The ideal candidate has at least 3 years of direct patient-facing experience, a multi-state LPN license, and the ability to coordinate resources for chronic care management. You will support providers in value-based care initiatives, manage medications, educate patients, and help ensure timely services like Home Health and DME.

Qualificações

• Active LPN license in multiple states (NLC/compact).
• Minimum 3 years of direct patient-facing experience.
• Ability to work remotely and maintain HIPAA-compliant workflows.

Responsabilidades

• Manage patient census with a resolution-driven approach to close gaps in care.
• Identify and coordinate community resources for patient care.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management, including reconciliation and refill coordination.
• Assist in timely delivery of services such as Home Health, DME, Home Infusion.
• Resolve patient questions and maintain open dialogue to assess needs.
• Manage referrals and appointment scheduling.
