Job summary

Responsibilities

• Provides individual and group interventions for mental health presentations.
• Provide cognitive screenings to address specific functional and/or cognitive ability questions.
• Provides immediate access to clinical assessment and collaborative care and treatment for those experiencing mental health symptoms.
• Provides psychosocial assessments to determine functioning and needs of patients and families.
• Provides DSM diagnoses to determine psychosocial functioning and needs.
• Maintains knowledge of Veteran benefits and services and makes referrals where appropriate.
• Provides case management throughout the continuum of care.
