Job summary

Trillium Health Resources in North Carolina seeks a Licensed Supervisor, Care Management - RCT to lead a remote-based team delivering integrated care within a managed care environment. Responsibilities include supervising staff, guiding care planning, and ensuring regulatory compliance while coordinating complex member cases.

The role requires NC licensure (LCSW/LCMHC/LMFT/LCAS/RN or RN with NC license) and travel within 75 miles of the South Central region.

Qualifications

• Associate's degree (RN only) with 3 years experience in care management, case management, or care coordination.
• Bachelor's degree (RN only) with 3 years experience in care management, case management, or care coordination.
• Master's degree and fully licensed in one of the listed licenses (LCSW, LCMHC, LMFT, LPA, LCAS, or RN) with 3 years experience in care management, case management, or care coordination.
• Fully licensed in North Carolina as one of: LCSW, LCMHC, LPA, LMFT, LCAS or RN.
• If applying with an Associate’s or Bachelor’s degree, must be licensed in NC as an RN.
• Must have a valid driver’s license.
• Must reside in North Carolina within 75 miles of Trillium’s South Central region.
• Must be able to travel within catchment as required.
• Must be legally authorized to work in the United States without sponsorship now or in the future ( OPT/F-1, J-1 ).

Responsibilities

• Supervise staff, including day-to-day responsibilities, performance management, development, and time/attendance.
• Ensure staff use established workflows to complete comprehensive assessments and care plans for members.
• Establish and maintain a team-based, person-centered approach to care coordination addressing physical, behavioral, and social health needs.
• Monitor team performance metrics, caseload distribution, and quality indicators for effective service delivery.
• Provide coaching, education, and guidance to staff for continuous improvement and professional development.
