Qualifications

• Must meet Qualified Professional (QP) status.
• Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN (NC) with four years MH/DD/SA experience.
• Master’s degree in a human services field with one year MH/DD/SA experience or substance abuse supervision experience.
• Bachelor’s degree in human services with two years MH/DD/SA experience or substance abuse supervision experience.
• Bachelor’s degree in a field other than human services with four years MH/DD/SA experience or substance abuse supervision experience.
• LTSS/HCBS coordination experience preferred; two years recommended.
• Immunizations: influenza, MMR, Varicella, Tdap; provide proof as required.
• Must reside in NC and within listed counties; travel within a two-hour radius of the Trillium region.

Responsibilities

• Engage members in care coordination/care management.
• Provide education about services, supports, treatment options, and diagnoses.
• Link to psychological, behavioral, educational, physical, and dental evaluations.
• Facilitate referrals to health services, community resources, and reintegration supports.
• Evaluate outcomes of care plans and reintegration efforts.
