Job summary

Trillium Health Resources is seeking a Licensed Supervisor, Care Management - RCT to lead a team delivering integrated, whole-person care in a managed care setting. Remote in North Carolina with potential travel within the 75-mile catchment and standard 8:30am–5:00pm hours.

The role requires licensure in NC (LCSW/LCMHC/LMFT/LCAS or RN) with 3 years in care management for MH/SU/IDD populations, plus supervisory experience. Competitive benefits and remote-work options are offered.

Qualifications

Associate’s degree (RN only) and 3 years experience providing care management to MH/SU/IDD populations.
Bachelor's degree (RN only) and 3 years experience providing care management to MH/SU/IDD populations.
Master's degree and fully licensed LCSW, LCMHC, LMFT, LPA, LCAS or RN with 3 years experience in care management.

Responsibilities

Supervise staff, including oversight of day-to-day responsibilities, performance management, development, and time and attendance.
Ensure staff utilize established workflows to complete comprehensive assessments and care plans for members.
Establish and maintain a team-based, person-centered approach to care coordination that addresses members’ physical, behavioral, and social health needs.
Monitor team performance metrics, caseload distribution, and quality indicators to ensure effective service delivery.
Provide coaching, education, and guidance to staff to support continuous improvement and professional development.
