Alliance is seeking a Care Manager II for a remote role based in Mecklenburg County, NC. You will lead care team communications, complete assessments, and develop unified plans of care for Tailored Plan members.

The role requires NC licensure (LCSW/LMFT/LCAS/LCMHC/LPA or RN), strong knowledge of Medicaid, and proficiency with JIVA. Travel to area counties and occasional onsite meetings are required.

Qualificações

• Master’s degree in Human Services and 2+ years of full-time MH/SUD and/or I/DD experience; NC licensure.
• Licensure as NC LCSW/LMFT/LCAS/LCMHC/LPA or RN with relevant MH/SUD experience.

Responsabilidades

• Lead communication among care team and serve as the primary contact for the member.
• Complete comprehensive assessments and develop unified plans of care.
• Coordinate with providers and community stakeholders to ensure continuity of care.
• Document activities in JIVA and ensure timely authorization.
• Travel within Mecklenburg and surrounding counties as needed.
