Job summary

CVS Health is seeking a licensed Behavioral Health clinician for a 100% remote Clinical Case Manager role across the United States. You will support members with complex behavioral and physical health needs through assessment, care planning, crisis intervention, and coordinated care.

Qualifications

• Master's Degree in Social Work, Counseling, or related Behavioral Health field.
• Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent).
• 3+ years of post-master's direct clinical experience in behavioral health settings.
• 3+ years of experience treating mental health and substance use disorders.
• 3+ years of crisis intervention experience and use of Motivational Interviewing.

Responsibilities

• Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs.
• Develop individualized care plans that promote improved health outcomes and overall well-being.
• Collaborate with providers, care teams, and community resources to coordinate appropriate care.
• Apply clinical judgment to identify and address risk factors, barriers to care, and complex health needs.
• Utilize Motivational Interviewing techniques to engage members and encourage long-term behavior change.
• Provide crisis intervention and ensure timely access to appropriate treatment and support services.
• Coordinate behavioral health and substance use disorder services across the continuum of care.
• Leverage technology and clinical systems to document care and manage member interactions.
