CVS Health is seeking a field-based Behavioral Health Care Manager to apply advanced clinical judgement in planning and coordinating care for members across behavioral health, medical, and community providers. The position requires a Master's in a behavioral health field and an active license (LCSW/LPC/LMFT/Psychologist preferred), with 3+ years' experience in behavioral health.

Qualifikationen

• Active, unrestricted licensure as an LCSW, LPC, LMFT, Psychologist, or other qualified behavioral health professional per state requirements.
• Master's degree in Social Work, Counseling, Psychology, Human Services, or a related field.
• Minimum of three years in behavioral health, case management, or care coordination.
• Experience conducting behavioral health assessments and developing member-centered care plans.
• Knowledge of mental health, SUD, trauma-informed care, recovery principles, and community-based services.
• Experience coordinating care across providers, families, and community systems.
• Strong communication, clinical judgment, documentation, and organizational skills.
