Job summary

CVS Health Corporation is seeking a licensed Clinical Case Manager – Behavioral Health to support members across the U.S. in a fully remote capacity.

You will conduct assessments, develop care plans, coordinate with providers, and intervene during crises, while using Motivational Interviewing and navigating virtual systems. This is a Monday–Friday role with no weekends or holidays, offering meaningful work and growth in a collaborative health care environment.

Qualifications

• Master's degree in Behavioral Health or related field.
• Active, unrestricted behavioral health license in your state (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
• 3+ years of post-master's direct clinical experience in hospital/ambulatory/outpatient settings.
• 3+ years treating mental health and substance use disorders.
• 3+ years using Motivational Interviewing techniques.
• Case Management experience.
• Ability to talk on the phone and document in clinical systems.
• Strong technical proficiency in a virtual environment.

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews.
• Develop individualized care plans that address both behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply advanced clinical judgment to identify and mitigate risk factors impacting member outcomes.
• Perform crisis intervention for members experiencing behavioral health or medical crises and ensure appropriate referrals and follow-up.
• Utilize Motivational Interviewing techniques to engage members and promote sustainable behavior change.
• Coordinate psychosocial wraparound services and connect members with community resources.
• Support cost-effective care through proactive care planning and resource utilization.
