Job summary

Graham Healthcare Group is hiring a Hospice Journey Counselor to provide information, education, and support to patients, families, and caregivers regarding end-of-life planning and care. This role may sit remotely if based in Troy or Lapeer, MI, with a hybrid work arrangement.

The counselor will manage patient referrals, assess eligibility for Hospice, and coordinate visits and communications across the care team to ensure compassionate, patient-centered care.

Qualifications

• High school diploma or GED is required.
• Prior home health or related healthcare experience preferred.
• Understanding of hospice philosophy is helpful.

Responsibilities

• Manage Journey patient census and referrals for the assigned area.
• Review patient charts for Hospice eligibility and preparation for discussions with patients and families.
• Make initial calls to new referrals to schedule home visits and follow up as needed.
• Conduct patient visits to discuss hospice benefits with patients and families.
• Maintain Journey patient charts and notes to document visits and contacts.
• Collaborate with Clinicians to address patient needs and ensure smooth transitions to Hospice.

Compensation: $19.00 - $21.00, plus bonuses

The base compensation range for this role is fixed, with a maximum cap of $21.00. We want to be transparent about this as we continue discussions.
Hours: Monday - Friday, 8AM - 5PM, Hybrid

Graham Healthcare Group is hiring a Hospice Journey Counselor to join our dynamic team! The Hospice Journey Counselor will provide information, education, and support to patients, families, and caregivers regarding end-of-life planning and care. Assist them in the transition to Hospice care when the patient is both eligible and appropriate for Hospice services. This position can sit remotely if you are based in Troy or Lapeer, MI.

Hospice Journey Counselor

Job Responsibilities:

• Manage Journey patient census and referrals for the assigned area from the Journey candidate list, the Journey Administrative Assistant, Customer Service, Clinicians, and other sources
• Review all assigned patient charts to determine eligibility and appropriateness for the Hospice benefit and prepare for conversations with the patient, family, and/or caregivers
• Make initial phone calls to all new Journey referrals to schedule home visits, as well as follow-up calls to maintain contact with the patient and assess patient decline
• Conduct patient visits for building a relationship with the patient and/or family, and in-depth discussion of the hospice benefit when appropriate
• Maintain Journey patient charts, documenting all Journey visits and contacts in Journey Visit/Phone call notes or as Coordination notes when applicable
• Build relationships with and maintain effective communication with Clinicians to ensure effective collaboration for addressing patient needs
• Ensure that Journey charts are created and maintained for each Journey patient
• Meet and exceed if possible, expectations for weekly patient visits, weekly patient phone calls, and monthly transitions to hospice care
• Assist clinicians in a more accurate understanding of the philosophy, goals, eligibility, and benefits of Hospice care
• Assist the Hospice and/or Home Health team in efficient transitions to Hospice in whatever capacity may be needed, whether in extra visits, calls, paperwork, or any other task

Hospice Journey Counselor

Qualifications/Requirements:

• High school diploma/GED
• Prior home healthcare or relative work experience in the healthcare industry preferred
• Understanding of the hospice philosophy preferred

Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
