Job summary

Molina Healthcare seeks an IRIS Consultant to support Wisconsin IRIS program participants from a remote home-based position. You will regularly visit participants in their homes and communities, building tailored IRIS plans for employment, housing, health, safety, and community involvement.

Successful candidates have at least 2 years in health care/care coordination, a relevant bachelor’s degree, and strong communication and budgeting skills.

Qualifications

• At least 2 years experience in health care, care coordination, or related field.
• Bachelor’s degree in social work or closely related field (or 4 years of commensurate experience).
• Valid driving license and reliable transportation.
• Ability to work independently with minimal supervision.
• Familiarity with IRIS or long-term care programs is a plus.
• Excellent communication and interpersonal skills.

Responsibilities

• Meet IRIS participants in person at least four times yearly.
• Provide program orientation and explain rights and responsibilities.
• Develop customized IRIS plans aligned with participant goals.
• Document activities in WISITs within 48 business hours.
• Coordinate with Fiscal Employer Agency for provider billing and budgeting.
• Attend monthly team meetings and participate in weekly check-ins.
