Job summary

The University of North Carolina at Chapel Hill is seeking a THRIVE Clinical Navigator to support the Transforming Health & Resilience in Veterans program. Based in Chapel Hill, this role leads intake, evaluates applications, and coordinates clinical work with the THRIVE team.

You will oversee scheduling, conduct interviews, and help determine program eligibility while coordinating with UNC Health protocols and partner programs. This is a full-time, in-person position with comprehensive benefits.

Qualifications

• Bachelor's degree or equivalent combination of training and experience.
• Experience with EPIC and data capture software (REDCap, Qualtrics, QuesGen).
• Experience with service members or veterans.
• Excellent organization skills to manage program timelines.

Responsibilities

• Lead THRIVE Program intake and scheduling.
• Oversee intake process and supervise Intake Coordinator.
• Review program applications, medical records and ROI forms for completeness.
• Conduct clinical phone interviews and complete online assessments.
• Create and present summary of assessment findings to Clinical Decision Team.
• Provide recommendations to team regarding patient’s needs.
• Schedule veteran and first responder 3-day evaluation visit.
• Register patients in EPIC and manage outlook appointments.
• Develop and maintain working relationships with partner programs.
• Oversee pre-screening veterans and first responders for eligibility.
• Assist with data information collection and entry.
