Responsibilities

• Assists families in determining needed resources and services through screening and triage.
• Answers questions and concerns and provides service referrals and scheduling.
• Gathers referral and pre-admission information and completes data entry.
• Identifies emergent or complex cases and seeks clinical review prior to finalizing referrals.
• Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.
• Uses Electronic Medical Record and Scheduling software to assess demand and service capacity.
• Maintains confidentiality of all clinical and administrative information.
