Job summary

UNC Chapel Hill is seeking a THRIVE Clinical Navigator to support the Transforming Health & Resilience in Veterans program. This in-person role will manage intake, scheduling, and assessments for veterans, coordinating with the Clinical Decision Team.

The position requires a Bachelor's degree or equivalent, experience with EPIC and data capture software (REDCap, Qualtrics, QuesGen), and strong organizational and communication skills.

Qualifications

• Bachelor’s degree or equivalent combination of training and experience is required.
• Experience with EPIC and data capture software is preferred.
• Experience with service members or veterans is desirable.
• Excellent organization skills and ability to manage multiple priorities.

Responsibilities

• Serve as Team lead for THRIVE Program intake and scheduling.
• Oversee intake process and supervise Intake Coordinator.
• Review program applications, medical records and Release of Information forms.
• Conduct clinical phone interviews and complete online assessments.
• Create and present summaries of assessment findings to the Clinical Decision Team.
• Provide recommendations to the team regarding patient needs.
• Schedule veterans and first responders for 3-day evaluations.
• Register patients in EPIC and manage outlook appointments.
• Develop and maintain relationships with partner programs.
• Assist with pre-screening veterans and data collection.
