Job summary

Modivcare Personal Care Services is seeking an Intake Coordinator with private-pay home care experience to support prospective clients from inquiry to successful start of care. This remote, work-from-home role emphasizes compassionate client service and coordination across internal teams.

The ideal candidate has 1+ year of direct home care intake or care coordination experience, private-pay expertise, and strong communication and organization skills.

Qualifications

• 1+ years of direct home care intake or care coordination experience.
• Private-pay home care experience is required.
• Experience with scheduling, staffing, and start-of-care coordination.
• Experience delivering high-touch service to clients and families.
• Experience with HHAeXchange, payer portals, or similar systems preferred.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Handle new private-pay inquiries and referrals with prompt, high-touch support.
• Conduct intake conversations to determine care needs, schedules, location, and start dates.
• Educate clients on home care services, rates, expectations, and onboarding process.
• Follow up with prospective clients to overcome barriers and move toward onboarding.
• Coordinate assessments, documentation, caregiver matching, staffing, and start-of-care activities.
• Maintain accurate intake records from initial inquiry to start of care.
