Job summary

Modivcare Personal Care Service is seeking an Intake Coordinator to ensure new client referrals are processed quickly and accurately, while supporting a positive experience for clients, caregivers, and referral partners.

Pay ranges $24.68–$26.50 per hour, with a schedule Monday to Friday, 8:00 AM–5:00 PM EST. Remote in MA. Join our team to help coordinate intake and onboarding, while building strong relationships with referral sources and internal teams.

Qualifications

• High school diploma or equivalent.
• 1+ years of direct intake experience.
• 1+ years of home care experience.
• Knowledge of Medicaid eligibility and payer sources required.

Responsibilities

• Answer and manage incoming referral leads through phone, email, and payor portals.
• Gather patient information, verify payer requirements, and obtain initial authorizations.
• Maintain accurate patient, authorization, and scheduling records in internal systems.
• Coordinate with Care Coordinators and branch teams to ensure smooth client and caregiver onboarding.
• Build and maintain strong relationships with referral sources and respond promptly to requests.
