Job summary

Pelago Health is seeking a Care Specialist for our digital clinic to guide members through intake assessments, welcome calls, and initial screenings. You will explain Pelago's care model and connect members with the appropriate ongoing care team, supporting a smooth care journey.

This remote US-based role is 70% onboarding and 30% ongoing caseload, with opportunities to contribute to pilots and quality initiatives. Must hold active state licensure and IC&RC/NAADAC credentials.

Qualifications

• 3+ years of clinical experience with substance use disorders.
• Active and unrestricted state credential (licensure/certification), issued via a state credentialing board.
• Multi-state licensure preferred.
• Credentialing recognized by IC&RC or NAADAC (e.g., AADC, CAADC, LADC, LCSW with SUD scope).
• Bachelor's degree in a relevant field.

Responsibilities

• Serve as the first clinical point of contact for members entering Pelago, creating a welcoming onboarding experience.
• Conduct initial screenings and intake assessments to evaluate behavioral health history, substance use, symptoms, and member goals.
• Understand Pelago care pathways and communicate treatment options to members.
• Provide continuity of care by connecting members with their counselor or coach and sharing relevant information.
• Maintain a small ongoing caseload with individual counseling and progress monitoring.
• Provide temporary member coverage during clinician leave or transitions.
• Document member interactions thoroughly and contribute to quality improvement.
