Job summary

Heartland Home Health, part of Gentiva, is seeking a Centralized Intake Coordinator to manage referrals across hospice, palliative care, and home health in a high-volume, centralized setting. You will handle electronic platforms, fax, phone, and email to ensure timely admission and care transitions.

The role requires HIPAA compliance, accurate data entry in multiple systems, and strong communication skills. Weekend 8am–8pm shift; remote collaboration is essential.

Qualifications

• High school diploma or GED required.
• 1–2 years experience in healthcare admissions, medical office administration, or contact center operations.
• Proficiency with computer systems and data entry; ability to learn multiple platforms quickly.
• Strong verbal and written communication; excellent organization and multitasking.

Responsibilities

• Receive, review, and process referrals across channels (electronic, fax, phone, email).
• Verify insurance and obtain authorizations for Medicare, Medicaid, and commercial plans.
• Enter referral data accurately in HCHB, Allscripts, and other platforms.
• Coordinate admission visits and DME ordering with clinical/operations teams.
• Track referrals, resolve barriers to admission, and maintain HIPAA compliance.
