Job summary

Vitality Hospice and Palliative Care is seeking an Intake Coordinator to serve as the primary contact for referrals and manage the intake process from referral to admission. Strong communication, organization, and attention to detail are essential for providing excellent patient service while complying with HIPAA.

The role partners with clinical leadership to ensure smooth admissions, accurate records, and timely patient care.

Qualifications

• High school diploma or GED required; Associate's or Bachelor's degree preferred.
• Experience in hospice, home health, healthcare intake, admissions, scheduling, medical office, or customer service preferred.
• Excellent communication, organizational, and multitasking skills.
• Tech-savvy with the ability to learn new software and EMR platforms; proficient in Microsoft Office.
• Knowledge of hospice or home health regulations is a plus.

Responsibilities

• Receive and process referrals via phone, fax, email, and electronic platforms.
• Coordinate with hospitals, physician offices, and case managers to obtain required documentation.
• Enter patient demographics, insurance, orders, and clinical data into the EMR.
• Collaborate with clinical/admissions teams to ensure timely admissions and scheduling.
• Verify insurance eligibility and ensure complete admission documentation.
