The role emphasizes building strong relationships with referral sources, maintaining census accuracy, and ensuring admissions align with contractual terms. A healthcare background and EMR proficiency are required.

Qualifications

• Degree in health care or related field preferred.
• 3–5 years of experience in health care, preferably in hospice operations.
• Admissions/Intake and customer service experience.
• Valid Driver’s License and Automobile Insurance.
• Knowledge of CHAP standards, government regulations, and private payer practices.
• Experience with EMR software.

Responsibilities

• Direct all daily patient referral and intake operations and help establish intake policies.
• Enter chart into EMR and maintain workflow.
• Maintain census, pending admissions, and care planning meetings.
• Ensure compliance with state, federal, and CHAP regulatory requirements.
• Collaborate with referral sources to promote Heart to Heart Hospice as a high-quality provider.
• Collect and report referral statistics and trends.
• Participate in strategic planning to meet unmet customer needs.
• Ensure patients are admitted according to contract provisions.
