Job summary

UDS Foundation seeks an In Home/Community Support Specialist to provide individualized, skill-building support in clients' homes and communities. You will work 1:1 to help develop daily living skills and promote independence within individual support plans.

Qualifications

• High school diploma or GED is required.

• Complete required documentation (daily notes, progress reports).
• Provide personal care as needed (toileting, mobility, feeding).
