Job summary

Creative Living Options seeks a Direct Support Professional – HSW to deliver person-centered, in-home and community-based support to adults with IDD. You will follow individualized plans, assist with daily living activities, and support mobility and safety while maintaining professional boundaries.

You will document progress, coordinate with supervisors, families, and healthcare teams, and participate in required trainings.

Qualifications

• Evidence of strong interpersonal and communication abilities.
• Ability to work well in a team and autonomously when required.
• Willingness to learn and implement specialized client-specific procedures.

Responsibilities

• Provide direct support and supervision per individualized plans.
• Assist with activities of daily living and community participation.
• Support mobility, transfers, and use of adaptive equipment as trained.
• Maintain documentation and communicate with the care team.
• Respond to emergencies per policy and training.
