Job summary

Blue River Services, Inc. is seeking a Direct Support Professional – In-Home Services to provide individualized, in-home and community-based support to individuals with disabilities.

The role focuses on promoting independence, life skills development, and helping clients achieve personal goals within their home environment. The position involves training in daily living skills, assisting with community access, documentation, and coordination with families and care teams.

Responsibilities

• Assess individual needs and assist in developing and implementing individualized service plans.
• Provide training in daily living skills, including personal care, communication, and social development.
• Support individuals in accessing community resources and participating in community activities.
• Assist with household tasks, organization, and maintaining a safe home environment.
• Escort individuals to medical appointments and community activities as needed.
• Support individuals with correspondence, budgeting, and bill paying when necessary.
• Maintain accurate documentation of services, progress, and outcomes in compliance with regulations.
• Develop materials and maintain supplies to support individual goals.
• Act as a resource to families, caregivers, and other team members.
• Promote independence, self-advocacy, and personal growth.
• Maintain positive relationships with individuals, families, and community partners.
• Remain flexible to meet the needs of individuals served.
