Meals on Wheels Western Michigan is seeking an In-Home Care Services Coordinator to support older adults in Kent and Allegan Counties. You will perform in-home and phone assessments, determine program eligibility, and help clients access meals and home-based services.

You will maintain electronic records, arrange referrals to community resources, report concerns, and ensure documentation is accurate. The role is remote with periodic in‑office meetings and trainings, reporting to the Home

Qualifications

• Valid driver's license and reliable transportation required.
• Strong communication skills, written and verbal.
• Excellent time-management and prioritization abilities.

Responsibilities

• Conduct in-home and phone assessments to determine program eligibility and service levels.
• Maintain electronic client records and track assessments.
• Assist clients with program navigation and referrals to resources.
