Job summary

Pima Council on Aging in Tucson, AZ seeks a Case Manager to provide in-home assessments and care planning for clients in Pima County. You will coordinate services, monitor satisfaction, and facilitate transitions between care levels as needed.

The role requires strong communication skills, ability to work independently, and adherence to program guidelines in a remote setting with travel to client homes. Bilingual English/Spanish preferred.

Qualifications

• Bachelor's degree in Social Work, Human Services, or related field or equivalent experience.
• 2+ years in case management or social services; 3+ years preferred.
• Must have reliable transportation and AZ Level 1 Fingerprint Clearance/background check (employer paid).

Responsibilities

• Travel throughout Pima County to client homes to conduct in-depth assessments.
• Develop care plans with clients and monitor satisfaction; adjust services as needed.
• Communicate with clients, families, providers, and community members via multiple channels.
• Maintain accurate client files and conformance with contract guidelines and reporting.
