Job summary

United Cerebral Palsy of Southern Arizona in Tucson, AZ is seeking a Case Manager for Individuals with Disabilities. This full-time role coordinates in-home and community-based services for the elderly and people living with all types of disabilities, with a schedule of Mon-Fri 8a-5p and occasional after-hours to meet client needs.

Qualifications

• 18 years of age or older
• Ability to obtain CPR/FA certification
• Ability to obtain fingerprint clearance
• Ability to clear a DES background check
• Ability to obtain a current TB clearance card
• Ability to work in a team environment
• Ability to handle difficult situations with clients/families
• Ability to work a flexible schedule
• Ability to learn and operate UCPSA program software
• Ability to work independently
• Ability to lift and transfer clients up to 50 lbs
• Own/regularly access a vehicle for visiting and transporting clients

Responsibilities

• Coordinating in-home and community-based services for clients.
• Scheduling shifts for clients and direct care staff.
• Orienting and training direct care staff with clients.
• Monitoring direct care staff performance and attending meetings.
• Conducting intake of new clients and quarterly home visits.
• Maintaining paperwork and documenting significant conversations.
• Ensuring client confidentiality at all times.
