Job summary

Institute on Aging seeks a dedicated case manager to perform comprehensive assessments and create client-centered care plans for older adults and adults with disabilities in Merced, CA. You will conduct home visits, coordinate services with Medicare/Medi-Cal, and maintain thorough documentation to ensure timely care.

Qualifications include at least one year of relevant experience, strong communication, and computer literacy; English/Spanish bilingual ability is highly valued.

Qualifications

• One year working with disabled adults and/or older adults required.
• Experience with medical and psychosocial problems of functionally impaired adults and older adults.
• Experience with mental/behavioral health diagnoses and substance abuse disorders is highly desired.
• Exceptional communication and presentation skills with clients, families and teams.

Responsibilities

• Perform comprehensive assessments and ongoing reassessments of clients’ psychosocial, physical and health needs.
• Develop and initiate a client-centered Community Living Plan in collaboration with clients.
• Conduct home visits and accompany clients to medical appointments as clinically indicated.
• Identify and coordinate appropriate community services and entitlements.
• Maintain care relationships, provide crisis intervention, advocacy and problem solving.
• Document all case management activity promptly and maintain charting for continuity of care.
