Job summary

United Cerebral Palsy of Southern Arizona is hiring a Case Manager for Individuals with Disabilities in Tucson. The role involves coordinating in-home and community-based services, maintaining a caseload, scheduling staff, and ensuring documentation and compliance.

Qualified candidates will have at least one year of direct care or case management experience, a related degree, and the ability to obtain CPR/FA and clear background checks.

Qualifications

  • Applicants must be 18 years or older.
  • Ability to obtain CPR/FA certification.
  • Ability to obtain fingerprint clearance.
  • Ability to clear a DES background check.
  • Ability to obtain a TB clearance card.
  • Ability to work in a team environment.
  • Ability to handle difficult situations with clients/families.
  • Ability to work a flexible schedule.
  • Ability to learn UCPSA program software.
  • Ability to lift and transfer clients up to 50 lbs.

Responsibilities

  • Coordinate in-home and community-based services for clients.
  • Maintain and grow a caseload.
  • Schedule shifts for clients and direct care staff.
  • Orient and train staff with clients.
  • Monitor staff performance and attend weekly meetings.
  • Conduct intake of new clients and quarterly home visits.
  • Attend meetings with other agencies involved in a client’s care.
  • Keep client files up to date and document communications.
  • Maintain client confidentiality at all times.
